INSTRUCTIONS FOR COMPLETING THE 2022
CVHS SCHOLARSHIP APPLICATION

- An application can be found here CVHS Scholarship Application 2022 (an editable Google Doc) If any question does not apply to you, use N/A to answer.
- The application must either be downloaded or you must make a copy to work on. Clicking the link above will automatically offer to make a copy for you. Please save it to your hard drive or Google Drive to work on. When you make a copy of a Google Doc, please add your last name to the title for ease of locating it on your hard drive or your Google Drive. Answer all questions. Leave no answer blank. After completing the application form, students MUST print a hard/paper copy to turn into the CVHS Administration Office. Electronic submissions will not be accepted. The application deadline is Friday, February 25, 2022.
- A picture (no larger than 2” x 2”) MUST be added to the document. The application will NOT be accepted without a clear, presentable picture of the applicant embedded into the application.
- The personal statement must be brief and include the information requested. DO NOT include your UC or other college essay as your personal statement.
- You may attach only one additional sheet if more space is needed to answer questions regarding activities and/or awards. Be sure your name is on the additional page (if attaching one).
- It is mandatory that, upon receiving an award, that a thank you note be written to the scholarship donor. This note must be neatly handwritten in ink on appropriate stationery and turned into the Scholarship Advisor. NO typed or computer generated thank you notes will be accepted.
SENIOR AWARDS PROGRAM – will be held on Thursday, May 26, 2022 @ 7:00 p.m. Any senior who wishes to have information regarding outside scholarships or college-based grants included in the Senior Awards Program, must submit this information to Mrs. Mulvoy in the CVHS Administration Office no later than Friday, May 13, 2022.

CSF seeks to identify, support and encourage higher education for students who have a high level of integrity, are committed, are self-motivated, and who persevere even in adversity.
