Opt-Out Form (Grades 11 and 12 Only)

The Every Student Succeeds Act (ESSA) requires that school districts provide, upon request, names, addresses, and phone numbers of all high school juniors and seniors to military recruiters, prospective employers, and college/university recruiters.
If you have a high school junior and/or senior and do not want GUSD to disclose this information, please complete an Opt-Out Form and return the form to your student's school office or to GUSD Student Support Services, Administrative Center, Room 203.
Note: This information is not requested for students in other grades. Please submit an Opt-Out Form only if your student is in 11th or 12th grade.
