Counselor/Work Permit Coordinator
All Work Permit requests will be processed by email. Please complete and email the Work Permit Application the to Mrs. Gonzales.
Work Permit Instructions:
1.Please complete the top portion of the Work Permit Application with your personal and school information and have your Parent/Guradian sign the applicaiton.
2.Your Employer must complete and sign the Employer section of the form.
3.When completed by all parties, please scan and email the form along with a copy of your Student ID or State issued ID to Mrs. Gonzales, Kgonzales@gusd.net. Please include your cell phone number in your email.
4.Once received, Mrs. Gonzales will issue your work permit.
Note: To be issued a work permit you must be in good standing with grades, attendance, behavior and be on track for graduation.
