You will need to bring the following in order to complete enrollment:

1. Birthdate verification (one of the items listed below)
   • Birth Certificate
   • Passport/Visa/I-94 Card (Arrival/Departure Record)
   • Baptismal Certificate
   • Hospital Record
   • Alien card
   • Citizenship certificate
   • Sworn Affidavit
2. Immunization record
3. Proof of residence (any two original, current bills listed below)
   • Gas Company
   • Electric Company
   • Telephone Company
   • Water Company
4. Name, address, phone number of an emergency contact (friend, relative, or neighbor)
5. Name, address, phone number of a family doctor
