2010-2011 Enrollment Form

Caregiver Affidavit - This form is only needed if a student is living with a relative, other than a parent or legal guardian, in the Glendale Unified School District.

Choice Permit - Parents or legal guardians of Glendale Unified School District resident students shall be notified, in writing, no later than April of each year, of the opportunity to apply for placement in a District school other than the student's school of residence. Acceptance of permit student is dependent upon availability of space.

Clark Magnet High School Lottery Enrollment - Clark Magnet High School opened in September 1998 offering a blend of basic curriculum with an emphasis on physical science, mathematics, and advanced technology to approximately 550 ninth and tenth grade students. The school is now a four-year specialized high school with over 1,000 students. A fair and equitable lottery is conducted by the Glendale Unified School District to determine qualified applicants. Students must be enrolled or eligible for enrollment in a Glendale District school. Students attending a Glendale District school on an Interdistrict permit are not eligible. Clark Magnet High School should not be considered a magnet school for highly gifted students nor is it affiliated with the LAUSD Highly Gifted Magnet Program.

Employment-Related Transfer Requests - Both parents, single parent, or legal guardian of students in grades kindergarten through eight must be full-time employees. One parent must be working within the boundaries of the receiving district. Employment must be verified. Acceptance of transfer student is dependent upon availability of space.

Interdistrict Permit - Students must attend school in the district where their parents of legal guardian resides. The Board of Education recognizes the need, and provides for, interdistrict transfers in special circumstances. The criteria for executing an interdistrict permit will be the same for students entering or leaving the Glendale Unified School District. Acceptance of permit student is dependent upon availability of space.

Intradistrict Permit - The Board of Education recognizes that parents may wish to choose a school of attendance other than the assigned school of residence. The Board therefore believes that parental choice among District schools should be included as an integral feature of a well-rounded educational environment. Acceptance of permit student is dependent upon availability of space.

Parents Living with Another Party - This form is only needed if parents and student are living with another family and do not have utility bills in their name.

Sixth Grade Program - The Glendale Unified School District has implemented a voluntary program for sixth grade students to attend Roosevelt, Toll, or Wilson Middle School. Students will be selected following a lottery proces {Insert File Link} s. Each participating school will have a number of seats designated, based on the size of their fifth grade class and the size of the school. Students must be eligible for enrollment in sixth grade in the 2010-2011 school year and must make a commitment to remain at their selected middle school for a minimum of one school year.
